ACCOMPANY DURING DOCTOR VISIT

1.The Customer or family member must provide accurate and necessary information about the patient to the Service Provider.
2.Appointment and registration information required for the doctor visit must be provided by the Customer/family.
3.All medical decisions, diagnosis, medication, tests, and treatment decisions are the responsibility of the doctor, patient, and/or family.
4.ZIVO or the Service Provider will not be responsible for the doctor's decision or medical outcome.
5.In case of a medical emergency, the Customer/family must arrange appropriate emergency medical assistance.
বাংলা ১. গ্রাহক বা পরিবারের সদস্যকে রোগীর সম্পর্কে সঠিক ও প্রয়োজনীয় তথ্য Service Provider-কে দিতে হবে।
২. Doctor Visit-এর জন্য প্রয়োজনীয় appointment ও registration-এর তথ্য Customer/পরিবারকে প্রদান করতে হবে।
৩. চিকিৎসা সংক্রান্ত সিদ্ধান্ত, রোগ নির্ণয়, ওষুধ, পরীক্ষা ও চিকিৎসার সিদ্ধান্ত Doctor, Patient এবং/অথবা পরিবারের দায়িত্ব।
৪. Doctor-এর সিদ্ধান্ত বা চিকিৎসার ফলাফলের জন্য ZIVO বা Service Provider দায়ী থাকবে না।
৫. কোনো medical emergency হলে Customer/পরিবারকে উপযুক্ত জরুরি চিকিৎসা সহায়তার ব্যবস্থা করতে হবে।
